Florida schools require an up-to-date physical exam for kindergarten entry and for any student enrolling for the first time. We complete Florida’s official School Entry Health Exam form (DH 3040) during your visit.
Beyond the paperwork, we check growth, vision, hearing, and general health, and review immunization records so your child starts the school year ready to learn.

A school physical is a general wellness exam required for enrollment. A sports physical specifically clears a student to safely participate in athletics. We offer both, and can complete them together in one visit if you need each one.

Yes — bring your child’s immunization record and we’ll confirm it’s current as part of the visit.
We recommend booking a few weeks before your school’s enrollment deadline, so there’s time to follow up on anything unexpected.
Many camps accept the same general physical exam form — check with your specific camp for their requirements.
We’ll note what’s needed and can point you toward getting caught up. The physical exam itself can still be completed the same day.
