Who Needs a DOT Physical?

If you operate a commercial motor vehicle (CMV) in interstate commerce, federal law requires you to pass a DOT physical exam. This generally includes drivers who:

  • Operate a vehicle with a gross vehicle weight rating (GVWR) of 10,001 pounds or more
  • Transport hazardous materials requiring placarding
  • Drive a vehicle designed to transport 16 or more passengers (including the driver)
  • Hold or are applying for a Commercial Driver’s License (CDL)

Many intrastate drivers are subject to similar state-level requirements as well, so if you’re unsure whether you need one, it’s worth checking rather than assuming.

What Does a DOT Physical Actually Check?

A DOT physical is a comprehensive exam designed to confirm you’re medically fit to safely operate a commercial vehicle. It typically covers:

  • Urinalysis (checking for underlying conditions like diabetes, not a drug test)
  • A review of your medical history, including medications
  • A physical exam of your heart, lungs, abdomen, and extremities
  • A check of your spine and range of motion

The goal is straightforward: confirm you can safely handle the physical and mental demands of operating a commercial vehicle, and identify any condition that could put you or others at risk on the road.

How Often Do You Need One?

In most cases, a DOT medical certificate is valid for up to 24 months. However, if the examining provider identifies a condition that needs monitoring — high blood pressure, for example — they may issue a certificate valid for a shorter period, such as 3 or 12 months, with a follow-up required.

What Can Cause You to Fail?

Very few drivers are permanently disqualified, but certain conditions can result in a temporary or conditional certification until they’re addressed. Common issues include:

  • Uncontrolled high blood pressure
  • Uncorrected vision or hearing below the required standard
  • Certain uncontrolled cardiac conditions
  • Uncontrolled diabetes requiring insulin, in some cases
